1. Permission to Participate
I, the undersigned parent or legal guardian, give permission for my child named above to participate in the event listed above. I understand that participation is voluntary and that my child is expected to follow all event rules and instructions from organizers and event staff. I further understand that participants age 10 and under must be accompanied and supervised at all times by a parent or legal guardian. Participants age 11 and older may attend the event without a parent or legal guardian present, and these participants will be under the general supervision of designated staff during the event.

2. Assumption of Risk and Release
I acknowledge that participation in the event may involve risks such as slips, falls, or other unforeseen incidents, and I agree that my child participates at their own risk. I waive, release, and hold harmless the event organizers, sponsors, volunteers, and venue staff, including but not limited to Champions Group, Inc. and its subsidiaries and affiliates, from any and all claims, liabilities, or damages whatsoever arising from or relating to my child’s participation in the event.

3. Medical Authorization
If an emergency occurs and I cannot be reached, I authorize event staff to seek necessary medical care for my child. I understand that I am responsible for any associated costs.

4. Media Release
I grant permission for photos or videos of my child taken during the event to be used for promotional purposes.
